I would just say something to the effect of:
A block of rooms are reserved at the following hotels:
Hotel 1 Hotel 2 Hotel 3
Be sure to list the hotel info including phone # and be sure to mention the name the room blocks are reserved under...
As for making them, I would just put this info the actual STD or just print it seperately on a sheet of card stock.

I have another question about the hotel info. How specific should you get? Do you include that there will be a free shuttle and give the actual rate per night? Should I include that it's a day wedding?

I would like to know that also! I think I would tell about the shuttle service but maybe just say a discounted rate rather than the actual room rate.
