How do I make my own favor tags....

I'd like to make square favor tags with my monogramt, with a simple 'thank you' on it .
Can you tell me how to do this, what I would need to buy from Michaels, or Staples, etc?

Or, what website do you suggest I use, that I can upload my monogram and create them?
I appreciate your suggestions. Thanks.
