formal etiquette says write everything out. if you choose to not write out 1 set of numbers, i think you should be consistent

written out or
7 smith street
apartment 5c

I'd do

Seven Smith Street
Apartment 5C

i would do
7 Smith Street

That's how we handled it on our invitations. Single digit addresses were written out, and apartment numbers were written normally.

bill at inkspun did

7 Smith Street
Apartment 5C

According to the thing I got from my calligrapher it would be:

Mr. and Mrs. John Smith
Five Jane Street
Apartment Five C
New York, New York 11231

Any number 12 or under is written out and your spell out the state name no NY it should be written out as New York.
