Formal written letter - give it first to your direct manager, then to HR. 2 weeks is standard.

This. I handed it to them in person and then explained the situation. The look on my boss' face was quite satisfying! I hated that job!

This is how you give notice.

'hi mr/miss boss. You have been overbearing and annoying. You dont know how to do your job and make me look bad in the process. You dont know what yourdoing. You make everyone feel like shyte and you bring moral down. So it is with great pleasure i leave this miserable hell hole and i wont look back. Not for one day, not for one second.'

'oh by they way, you might be getting a call for a reference. I know I can count on you to give me a good one'.

Everyone gave you the right advice. However, one thing I want to add is that in your exit interview (which is customary nowadays), try to refrain from any super negative things. You're leaving, you're done with the place...but they can always give you a wicked reference because of the things you say. No, they're not SUPPOSED TO do that, but they can. Although it's temping to vent and get it out, the important thing is, you're leaving for whatever reason. Just leave it at that and walk away with your head held high.

Give a written letter to your supervisor and they will inform HR, with at least two weeks notice but make it so that you get a least a week between leaving your old job and starting the new one.
Sometimes HR will offer an exit interview, which you shold take ad vantage of.
