I am changing my last name to FH's. I was wondering, how you go about changing it on everything! Example: Social Security Card, License, Passport, Credit Cards, etc.................. Thanks!s

i went to motor vehicles with my marriage license and birth certificate and filled out a form - it was very easy. I still have to go to Social Security - they have a form too. Just bring your marriage license and other proof of identity such as birth cert and social security card.

I ordered a whole name change kit that included all the forms pertaining to the state you would be living in, like DMV, SS, credit cards, car leases, passports..I think this site them..I don't remember the site i ordered them from, but i will find out for you.

i went to DMV & SS office the same day. Be sure that if you own a car to change your title and registration when you do your new ID. The ID cost me $15 and has a new photo so come prepared for a picture. If you're changing the name on the title, bring the Old title.
SS was the easiest. You download the form and walk in with it. Bring your marriage cert to EVERYTHING you do.
You give up your SS card and get a new one in the mail. They punch a hole in your old license and you get a new one in the mail, your title is mailed and registration sticker is handed to you.
Bank: I went TO the bank for the name change and had to show my marriage cert. One account I always had joint with Jim so I had to bring him with me for that one. (although actually he went to all this with me cause if I have to go through this, so should he).
Office HR asked for the marriage certificate although for now (until next year's wedding) I still go by my maiden name. My benefits and paycheck have my married name (for tax purposes).
Insurance: Had to call my insurance folks and let them know it was official. Which adds Jim to my car (we also just added his car to my policy cause my rate was better) and added Jim's property to my Homeowners policy. Unfortunately since I live with him and his family it also adds his screwy brother to my auto policy until I can fix that (working on it now)
Credit Cards: Calling them up as I remember. But most places send you bills and on the back it says Changes?? and you just fill in the new info. I am doing this one at a time so I can wait for replacements and keep using my maiden name on most.
What else.. Well I got married (civially) on Oct 23, 2003. So far this is all ive done heh. I can update as I do more.
ETA: still need to work on change my name on memberships i have and associations Im involved in (Ie AAA and my Professional designations)

You can order a bride name change kit. It costs 24.95 but I found it very helpful. It came with all of the forms you need and gives instructions on how to fill them out. www.kitbiz.com I wouldnt change your name until after you are married and get back from the honeymoon.
