How many readings for Catholic Ceremony (not mass)?

I was looking at my booklet last night and it looked like the second reading was optional...did I get that right? I got that impression from the form I have to give the music director (it said Second Reading: Yes or No)

So there's the first reading, then the vocalist sings the responsorial, then you can have a second reading, then the Gospel/Homily.
Then there's the 'Lord, hear our prayer' reading (that's called a Responsorial Psalm right? You'd think I'd know this having gone to Catholic school but I don't really go to mass anymore!).

So...you can have three, but if I want I can option out of the second reading before the gospel, right?

I just have to figure out how many readers we need and who we should choose.

TIA!
