I think you should write:
Mr & Mrs John Smith & Daughter

The outter envelope I addressed it simply Mr and Mrs __ __. Inner I used all their first names i.e. Vincent, Mary and Alyssa. If you're only doing outter I would separate the names

Mr and Mrs John Doe
Miss Jane Doe.

Children
Child under age 18 Do not put on outer envelope.
Only list the parents. and Kaitlyn (1 child)
Kaitlyn and Johnny (2 + kids)
List by age, oldest first

Child age 18 +
should have own invitation Miss Kaitlyn Jones Miss Jones (and Guest)

SORRY...I thought that there was an inner...I've got wedding on the brain.

I would just do
Mr. & Mrs. John Smith and Family

If no inner envelope, then I say

Mr and Mrs John Doe
Miss Jane Doe
ADDRESS

don't worry, I am not using one either
