How to do the little note for the almonds???

Okay, excuse me for being so non-creative, but I am clueless!
I know that some brides on here have done this or are doing this because I remember the threads that discuss the little poem that goes with the 5 jordan almonds (mostly for Italian weddings i guess).

How do you make the little note?? On a little tag or a note card or what? if it is a tag, how do you attach to the little thing of tool?? (we are using tool to make little pouches)

Any help would be appreciated, thank you!

I didn't want the almonds but my mom came up with a really cute idea for them.

She found these little wine glasses (about the size of a shot glass) at the 99 cent store. She put five almonds in tuelle and tied them with ribbons with our name on them and stuffed them in the glass and put the little piece of paper with the Almond Poem. Its so cute. I wish I had a picture of it. We are putting one of these glasses on each place setting at the rception.

use an avery label as a template, write your names and date and then print out and cut and then use ribbon, in your color(s) and tie it..

here's a poem:

Five sugared almonds for each guest to eat
To remind us that life is both bitter and sweet.
Five wishes for the new husband and wife --
Health, wealth, happiness, children, and a long life!
