I am pretty sure it's formal but I also think you can choose to do it however you want. Our place cards are from our RH, so they actually have the M___________________ already there, so we're just keeping them formal.

we did ours 'Mr and Mrs' unless the couple was not married and then we just put both of their names but still used 'Mr, Mrs, Ms, etc'

but u can really do whatever u want....its your wedding...you make the rules

We did Mr. & Mrs. for couples and for singles (example: Mr. & Mrs. John Smith OR Ms. Lisa Smith) UNLESS it was an unmarried couple w/ long names! If I couldn't fit them both, I just dropped the Mr. & Ms.
I personally don't think it matters either way... whatever you want to do is completely fine. Nobody will notice!!!!
