Ours was the same as Regy's

Our DJ had a whole list of choices for me to pick from at the final meeting

I like this choice. Our DJ just has a sheet with a blank line for me to fill out.

My sheet had a blank line on it too, but then when I had my final meeting my DJ went over the different choices of introductions

I just feel weird about my first name not being in there... it;s like ok I'm giving up my last name, fine, but just to be introduced as 'Mrs.' kinda irks me. I feel like I have to be in there somewhere.
