You just need proof of age (license or passport is fine) and 2 forms ID

These may include any 2 of the following:
Driver's License
Non-Driver's license
Passport (Valid) - Foreign passports must have VISA
Naturalization Papers (original only)
US Military ID
Employer's Photo ID

Here's the website I got it from:

I got mine in Queens, but I needed a MONEY ORDER - they would not take cash or a check. I also only needed one piece of ID, my passport.

I just got mine on Friday. Both of you have to be there. One girl got turned away because she came by herself. And you need to know where you were born, or what hospital. My FH couldn't remember the name of the hospital where he was born.

I think it differs from place to place. For instance, in the 5 boros of NYC, you need a $35 money order and one piece of ID. Tehy didn't care where we were born (except for on the form) and we didn't need birth certificates - they just took the passports and that was that. In Hempstead, you need $40 cash and 2 pieces of ID. Call the office you plan on going to and find out for yourself.
