I NEED ETIQUETTE HELP!

I got a wedding invite with an RSVP that's a blank card.

So, Is:

'Mr. and Mrs. John Smith will attend the wedding of Jane and Adam with pleasure'

The proper response? Or, should we address it to the hosts and sign our names:

Dear Mr. and Mrs. Jones and Mrs. and Mrs. Stevens,

Mr. and Mrs. John Smith will attend the wedding of Jane and Adam on April Ninth with pleasure.

Sincerely,

Mr. and Mrs. John Smith.

I've never seen that before. Is it normal to include just a blank card?

I like Mr. and Mrs. Smith will attend with pleasure.

When I was looking at invites the one that I picked came with a blank card. I changed it to another reply card because I've never seen one and thought most guests wouldn't no what to write. Maybe a blank card is more popular in other parts of the country?

Actually, blank cards are very 'vintage.' That is what they used to do back in the old days. My cousin did that for her 1920's themed wedding. She did everything by 1920's ettiquette.

ETA: when i responded to hers I just wrote a little note to her and her FH. They loved getting everyones responses and some people wrote some really sweet things to them.
