I believe that the initials included are your FH's first inital and your first initial and then your new last initial, which I think is usually in the center.

a monogram is the Brides first initial, then the BIG last name of the groom (that the bride is taking) followed by the grooms first initial.
so if it was mary and john smith it would be

cake topper you mean?? well we are just doing the last first initial of our married name.. or you can do your first initial of your first name and his first initial and then the third letter will be bigger and that would be your new last name... I hope that makes sense
