Invitation question regarding reception card...help please!

I want to write this:

Adult Reception, Dinner and Dance
half past six in the evening
Jericho Terrace
Mineola, New York

My questions are:
1) do I capitalize the 'h' in 'half past...'
2) do I write out the address for JT?

I would capitalize the H (JMO) for consistency

You do not need to include JT address if you are including the directional card with the address
