When you have a couple that are engaged or living together how do you address the invitations?

Ms. Joan Smith and Mr. Mike Hall

or

Joan Smith and Mike Hall

For my friends who are engagaed but not living together, I put the person's name that I knew on the outside envelope:
Mr. John Smith
and in the inside envelope I wrote
Mr. John Smith
and
Ms. Jane Doe
