invite addressing question - weird

ok... so I have searched everywhere but I just cannot find how to word this. My cousin is single but has 3 kids. My invites are pocket folds so there is no inner envelope. I just don't know how to address the envelope and invite him with a guest (he may or may not have a girlfriend to bring) and with the kids:

Mr. John Smith and Family and Guest ???
or
Mr. John Smith, Family and Guest ???

It looks long and weird to me. I am just looking for the appropriate way to word this.

I will do the same then. It just looks weird cause his address is so short. Thanks for the help!
