Invite question - Please help!!

I figured I'd take advantage of being snowed in and start typing up the name and address list for the printer. I am not having inner envelopes, so I am very confused about how to address the outer envelopes! SO my questions are:

If there are young children in the family, is it acceptable to address it to 'The Smith family' ? Or should I list each guest separatley on the outer envelope?

Can I write 'John Smith and Guest' on the outer envelope if I don't know who'll he'll bring? (No steady GF)

Is it weird not having inner envelopes? I probably have time, should I order them?!

I'm so confused! Please help!

On the outer envelopes, if we invited the kids, we put, Mr. & Mrs. John Smith & Family.

If I was inviting with a guest, it was Mr. John Smith and guest.
