LABEL Help please

Okay here is the issue, I need a label approximately 5 x 5 and I want the return address printed and the bottom half I will have the calligrapher complete. I can't find where to get the labels made in this size and it has to be a label the calligrapher can use(np Gloss). Help anyone with suggestions.

Thanks Ladies
