legal advise

Well its not so much legal advise but I need to write a letter to Family Court IN QUEENS
I have a on going child support file going on out There and would like to have it moved to Suffolk. ALSO THE 'father' has filed a petition to have visitation right AND I need to move that to SUffolk also being that is our current residence for over 2 years

AM I asking for change of Venue? OR jurisdiction??
I am going to enclose proof and send a copy to both QUeens and SUffolk family court
I would just like the letter to sound intelligent and use all the correct terminology
If any one can help me with a template or if they know of one on line that would be greatly appreciated

I looked all over the web and I couldn't find anything for NYS. I did find something about changing jurisdiction for Michigan but that doesn't help you.
