does anyone know what to do if u lose that? im going to need it for passport and marraige license ...help! thanks !! im assuming i would call up the hospital i was born in to get a new one but im not sure if thats what i need to do???
Call the hospital or the county you were born in. They should be able to help.
It's super easy....my fh just had to do it for our passports. You go to the Department of Mental Health and Hygiene (or something like that) in the city and you can get one for $15.
you will have to go to the town you were born in. i dont beleive hospitals give them out anymore.

I got mine from town hall where I was born. It was only $15 and takes a few minutes. I think I actually downloaded the form online and my Dad brought it in for me. Call the offices in the town that you were born and they will tell you exactly what to do.
