i did... designed one in MS Word and printed it on velum paper and then glued it to a votive candle holder

I didn't make it myself but got one from etsy for about 20-25 dollars.

Mine is in my album. If you are up to it, i would definitely suggest trying it. Just print your design on vellum paper.
Usually I use a horizontal 8.5 x 11 template and center the design on the page, then print...

then wrap around the candle and add your ribbons. the thing that gets tricky is gluing it. stick glue would be the best but you have to get a strong one
Try the UHU sp? glue. the stick is purple and the outside is yellow
