When do i need to go to get this? Do i need to call in advance so they have it for us? Or can it all be done right then and there?

We just walked into brookhaven town hall, its off of n ocean in farmingville. They are open until 7 on mondays , and I think 5 the rest of the week. No need for an appointment you just walk up to the desk & fill out a form it took 20 minutes. Also its only good for 60 days so don't go too far in advance.
