It depends where you are going. We went to Smithtown and needed only SS card and drivers license. Alot of places also require birth certificate. Also, the fee is $40. I'm not sure if there are restrictions on how you can pay. We just paid in cash.

We went to the town of hempstead they needed photo id. And a 2nd form of idea. No SS card. But they accepted my FH health insurance card???? I didnt have this so I had togo across the hall and get my birth certificate which was 10.00. the license cost us 40.00
