In Nassau you can't fill it out online. But you need your birth certificate, drivers license, and $40.

In Queens you don't need your birth certificate.
We just had our licenses.

i have awhile until i need to do this, but....
do i get it in the county I'm being married? live? or doesn't matter
we live in nyc, but getting married in nassau. does it matter?

I don't *think* so. It's a NY State license so I don't think the county should matter, but don't quote me on this!

just checked....its good for NY state...
Duration of the Marriage License A New York State Marriage License is valid for 60 days (except for active military personnel, for whom the validity runs for 180 days). A Marriage License issued in New York can be used anywhere within New York State but may not be used outside New York State.
