Marriage License help

Is it just me or am I completely missing the application on the website?

I don't see it and I know that some of you girls said you filled it out ahead of time.

dont even worry about filling it out ahead of time. it seriously takes like 15 mins. we were in and out!

I'm not sure~I live in Queens and that's the one I submitted
