You need an original birth certificate (raised seal) and a DL or Passport, if you go to certain locations (Smithtown, for instance) you may also need your social security card.

if u go to the town of hempstead, all u need is a driver's lisence and passport.
no birth certificate needed....

We went to town of Brookhaven and needed a certified copy of Birth Certificate or a Certified baptismal certificate and a DL.

about 4-6 weeks
