- FH's baptismal certificate
- My baptismal certificate (does he keep the original???)
- My Pre-Cana completion form
- Church offering

You need to get a baptismal certificate that has been issued in the last 6 months, not the original

nope...you have everything you need. you should also have your readings and song choices if you haven't notified them already.
