Do you have to make a menu or can the waiter just tell everyone the choices. What do people usually do?

You can do either! I would rather look at a menu personally - gives me more time to decide what I want!

I'd say it's about 50/50...

You don't have to have a printed menu... It's a matter of preference and budget...
