Menu help

I am having menu made up soon, and I was wondering if you guys could show me ideas of what yours looked like?

the actual menu, I am not sure how I am supposed to have it all layed out. Do I show if there is a kids meal option, and if people are offered a second option should I add that?

This is the best pic I have of my menu.
Listed the courses seperately, laid it all out in word.

My pic won't work...but we have it laid out as:

Soup
(type of soup)

Appetizer
(type of app)

Choice of
then the four entrees

Dessert
(list of dessert...cake,venetian display, chocolate fountain--each on a separate line)

And our name is at the bottom
