There is a place in New Hyde Park off of Denton Avenue. I'm sorry, I don't know the name but that's the place I usually go to. There are two courses.. Romeo and Juliet.

Eisenhower Park and Jones Beach also have mini golf.
There is one up in Bayville. Also one in Lawrence/5 Towns, and one on Hempstead Turnpike in Bethpage/Seaford
