Names&Addresses for STDs... w/ guest?

So for my singles, if I know I'm inviting them 'with guest' for the wedding, do I put 'and guest' on the STDs?

No I dont think thats something they need to know that far ahead of time I wouldn't worry about it...

just addressed mine tonight! unless they are living together and being invited as a couple nope - even the ones in relationships werent addressed with and guest for now. never know what might happen!

I didnt...unless they were in a relationship and living together it was just the guests name
