need wish box help! PLEASE!

So a while ago in one of my many bridal mags i saw this article about wish boxes...it was giving you ideas on how to set it up for people...like instead of leaving the sheets of paper blank for them to fill out, it had a phrase or something at the top of the card and then the guest would fill it out from there...i went thru all my mags and cant find it anywhere did anyone see this? does anyone have any ideas?
