NEEDD HELP

we are going to meet with our deacon tonight and I totally forgot to fill out the together for life book and it's at fh house in li and the deacon is in bklyn ad there is no time to get it..

Does anyone have it and could outline what i need to give to the deacon tonight..

i actually brought it with me today, but its like a 100 page book so i cant type all that.
