NWR: another really DIFFICULT puzzel for you smarty pants

The nickname of a fiftieth of the United States, when spoken aloud, sounds like a short sentence meaning 'Sitting Bull's tribe medically cared for an octet of people.'
When you think of this nickname (composed of a six-letter word and a five-letter word), take the first letter of the place to which the nickname refers.

this one I CANNOT get! I need your brains!
