parents on program

What is the proper etiquette for addressing a deceased parent on a ceremony program? It somehow feels wrong if FH's dad was not included under groom's parents.

Also, FMIL has a boyfriend she's been with forever and I'm not sure if I should include his name somewhere in there. FH considers him to be a stepdad, but he's not even married to his mom, plus they've been having major problems lately that may lead to a breakup. He did pay for our invites though...
