Personalized dress label?

Where did you girls get that personalized label for your wedding gown? I've seen them before on some of the professional pictures. I was wondering how do I get one for my dress?

TIA!

When peeping at another LIW bride's album (thanks Jennie0898!!!) I saw a few customized things I liked and she told me she got them from DJ Chic- I was looking at ring bearer pillows, but I saw she also has dress tags.

Here's her website:
DJ Chic
