I would like to have a candle as a place card. Have the table number and name... Has anyone seen this? If so do you have any other place card ideas for a summer wedding?

I haven't seen one as a name card, but you could use the same idea as a personalized water bottle - put the table number and name on it, wrap it around the candle and put all the candles lined up on a table.
oops, made mistake with table and name...
As for the summer theme, maybe you can put the fake grass under the candles?
I really like the first and last one... Do you know where to order them?

I haven't seen a place online to order them. I am going to make it a DIY. I was told by the ladies on the 08 board you can buy the stem votive holders at acmoore. the regular votive holders you can get at any craft store. I've heard its easy to do... just print the info on vellum paper ...cut it to fit and then tape or glue t to the votive holder. HTH

Find out first if your RH will let you do this. Not all do. An open flame may be a liability issue for them, guests carrying them around with drinks and food.
