Placecard wording

For the placecards, do you write out Mr and Mrs? or first names?

I plan on being more formal with Mr. & Mrs... If someone is bringing a guest, it will be Mr. Doe & guest (in the event that at the last minute they switch who they're bringing...)

I would write out complete name...Mr. John Doe and Guest...or Mr. and Mrs. John Doe

Since I never want to be know as Mrs. Husband's first name husband's last name, and most of my friends also hate that I plan on having:
Bill and Cindy Smith on mine.
I also did that on my inner envelopes of my invites.
And for those invited with guests I will find out the guest's name and use their name instead of guest.
