Places to Change Name

Does anyone have a complete list of places you need to change your name with, and any specific forms you need to use?

ssn and liscense are the biggest ones to worry about.... then you have your banking companies you deal with, passport...pretty much anything that has your name on it. some require copy (or real) of your marriage certificate.

you have plenty of time though, i wouldnt stress about it.. enjoy being married. fw just started about a month or two after being married changeing things. and we are still not done...we didnt do credit cards yet...
