Putting together my invitations?

Am I supposed to put the response card inside of its' envelope or do i leave it out with the other cards?

Here's what the knot says (see right-hand side bar): Knot Article.

. Slide the reply card faceup beneath the flap of the reply envelope.

2. Set the reply card and envelope facedown on the reception card.

3. Set the reception card faceup on top of the invitaiton.

I have a related question: if you have a pocketfold that doesn't hold all of your enclosures, do you put them in front of, behind, or inside the flap of the invitation?
