The MOH comes in right before the bride. As the MOH walks into the church, the church doors close behind her. When she reaches the alter, the organist begins the bridal song and the doors open and then the bride walks in.

I had MOH walk then the ring bearer, then the flower -girl, then me and dad.

At my sister's wedding, I was MOH and I walked last, BEHIND the FG and RB, it cna go either way, but the RB was really young, so if there were any issues, I was right behind him to grab his hand and address any issues. So the kids were sandwiched between two adults. Felt safer..Luckily there was no conflict at all anyway...except that the RB who was 4 kept making Spider Man hand movements (like he's shooting a web) in all of the pictures...At first we thought he was giving people the horns! LOL
