Question about addressing envelopes

Has anyone run into the below situations and if so what did you do? If not what do you think i should do

I am preparing the address for the STDs. What are your thoughts on this situation...

Married couple but the wife uses her last name...

Mr. & Mrs. John Doe and Jane Smith Or Mr. John Doe and Mrs. Jane Smith?

And what if both are doctors, married, same last name...

Drs. John Doe Or Dr. and Dr. John Doe Or Drs. John and Jane Doe

I have a few girl friends that keep their last name so I asked them what I should do and they said this:

Ms. Jan Doe
Mr. John Smith
