Question for those who used the inner envelope

If I invite my friend with guest, does the 'with guest' go on the outer envelope or the inner? I'm so confused. Which one looks right?

Outer:
Ms. Christina Jones

Inner:
Christina & Guest

OR

Outer:
Ms. Christina Jones & Guest

Inner:
Christina & Guest or just Christina

This is how I think it should go:

Outer:
Ms. Christina Jones

Inner:
Christina & Guest
