Response card wording help

Okay, so I want to add a more personalized touch to my wedding and I'd like to have my guests write out their complete names on the response cards.. how should I word that?

NAME(S) of GUEST(S) ATTENDING:

how?

We left it up to our responders. At the top it said 'The favour of a reply is requested by September 15th' and then we left the card blank. Some people simply wrote 'Mr. and Mrs. John Smith will attend,' others wrote really nice personal notes to us. I liked that!

If you want to use that format, the first one is the best with (s). I'm no expert but it seems more grammatically correct. I do prefer Stacey's format, I think people will write in their full names. I always do when I have a response card in that style.
