If a mom and daughter both attended the shower, and gave you the gifts together, is it okay to send them one thank you? (if they live together, that is...)

If they lived together i put them both on one card but if they were seperate, i did two seperate cards.

if one household bought one gift I wrote one thank you to multiple people but if one household bought separate gifts, I sent each of them their own thank you

Yes. I had my thank you notes attached to the favor- in that case they would of only received one favor.
