Silverware paper punch - where can I find one?!

I found a super cute idea on Pinterest for wedding menus...but I cannot find the silverware paper punch!

This: http://www.scrapbooksupplyhouse.com/silverware.html
is the item but it is unavailable on this site.

Anyone have any ideas/suggestions/one I could buy from you!?
