Did anyone mail in the paper to change the name on your SS card? I found the form, but I'm not sure about sending documents with it. Can anyone let me know what they did if they mailed in the document?

If you mail it, you have to mail your original documents. If you do they will mail the documents back to you when they are finish verifying it.
you can go to the office and they will photocopy the original and give it back to you right then and there and they will stamp the copy that they saw the original
