I ordered a dress by Casablanca from RK Bridal and I absolutely LOVED it when I tried on the sample. The problem is, I am having an Orthodox Jewish wedding so my dress has to be VERY conservative, meaning high neckline and sleeves. Casablanca is awesome and they will build your dress up to your specifications when you order it so they are adding 3/4 length sleeves and a high round neckline. I am so bad at visualizing the dress with sleeves, so I have been nervous about it for months. I got the call on Thursday that the dress (sleeves and all) is finally in and I made an appointment to try it on on Monday. But I can't stop freaking out about it!! Here is a pic of the dress how it's supposed to be (I plan on wearing it without the detachable bow):

2nd I feel your pain about visualizing...I need to to the same with my dress but it's tough.
Everything will be absolutely gorgeous though

i kind of know how you are feeling - im doing a change to the neckline of my dress which i think will give the dress a totally different look - of course i have no idea what it will actually look like until my dress comes in! its really nervewracking!! but at least you will get to see it soon! im sure wit will be perfect!

honestly the word they do is outstanding. I had a custom balero made for my dress by them with out even knowing what it looked like and I didn't want to take it off the whole night. Your going to look amazing!!!

the detail is gorgeous -- will the sleeves be detachable and additional neckline be detachable for the reception? Can that be done or am I nuts?

It's lovely, stop doing what many of us keep doing, doubting and second guessing. I am on my second dress as it is. Ha ha. Also, these people are most likely professionals to still be in biz, right? They'll do a great job I'm sure. But yes, I know how you feel.

Nope, won't be detachable! It will be one continuous piece of fabric.
