Sponsor Certificate for Christening...Queens Church

I am going to be godmother to my niece in March up in Stamford and they asked me to get a sponsor certificate from my church. Ummmm, I havent been to church besides holidays in years....what does this certificate consist of? Will they ask where my $ is when I go for certificate? What does this involve?

Has anyone had any experience w/ churches in Queens & what it entails?

Thanks.
