Sponsor Sheet for Godparents Church...HELP!

I posted on NWR, but no responses....Figured you guys could help.

I am going to be godmother to my niece in March up in Stamford (YEAAAAAAAA!) and they asked me to get a sponsor certificate from my church. Ummmm, I havent been to church besides holidays in years....what does this certificate consist of? Will they ask where my $ is when I go for certificate? What does this involve?

Has anyone had any experience w/ churches in Queens & what it entails?

Hmm my church never asked for this from the Godparents In fact they told us what the requirements were ie, one had to be female and one had to be R.C. etc... but never checked eithers credentials! LOL

I had to get this when I was godparent to my nephew. Just call the local church and explain what you need to get. If you didnt make your sacraments there they may want to talk to you first before giving it out! Which is what happened with my brother b/c he lives like 100 miles from our church!
At my church there was no fee, I came in they signed it and I walked out, it wasnt a big deal at all!

This is something that is new and not all churches ask for it... I dont think it involves a fee. Just call your local church.

Not all churches/priest ask for it,
I have been a Godparent 2x,
first time they didnt ask for a certificate,
second time they did,
I called church I got married in and told them what I needed and my name, and they just told me when to pick it up.

Are you getting married in a church?
if so call that one for it,
